Job Title: Home Health Physical Therapist
Job Specialty: Therapy
Job Duration: 13 weeks, with the option for a longer term of up to 1 year
Shift: Flexible schedule, typical is Monday-Friday 8:00am - 4:30pm, with options for Sunday-Thursday or four 10-hour days
Guaranteed Hours: 40 hours per week
Experience: New graduates welcomed
License: National registration and licensure for the state of practice required
Certifications: Completion of an accredited physical therapy program
Must-Have: Ability to effectively communicate in English, both orally and in writing
Job Description:
- Assess clients' needs and develop therapy plans that meet identified needs.
- Administer and perform therapeutic interventions with clients.
- Maintain clients' self-esteem, privacy, and confidentiality of personal information.
- Provide clients with opportunities for independence and choice consistent with their care plans.
- Notify physicians of changes and process orders accordingly.
- Document observations and services provided in clinical records.
- Provide direct consultation with physicians and/or interdisciplinary team members, exchanging necessary information.
- Train individuals on programs, adaptive equipment, and other interventions.
- Utilize and supervise therapy assistants as outlined by Federal and State regulations.
- Adhere to appropriate bag techniques and standard precautions, following infection control and safety control standards.
- Take responsibility for complying with all related laws, regulations, company policies, and procedures pertinent to the position.
- Supervise assigned employees, including interviewing, hiring, training, planning, assigning, and directing work.
- Appraise performance, reward and discipline employees, address complaints, resolve problems, and foster employee engagement.
- Reasonable accommodations may be made to support individuals with disabilities in performing essential functions.
- Moderate noise level work environment with varying travel frequency based on business needs.
- Weight lifting requirement of up to 50 lbs.
- Drive time and mileage paid between facilities and clients' homes.
- Partial split between home health and skilled/long-term care based on greatest need.
- Support available from Area Manager and Clinical Specialist.
- Software transition from Net Health to Home Care Home Base in January.
