We are currently seeking a Home Health Program Manager to lead our Home Health department.

SUMMARY
This position will assume overall responsibility for oversight and management of clinical and operational services provided by the department of Home Health including core and non-core services.
RESPONSIBILITIES
- Assumes overall responsibility for the delivery of care provided by the staff of the home health program
- Demonstrates a working knowledge of the CMS regulations, the Conditions of Participation, Joint Commission standards and the community standard of provision for home health care services and can exemplify that by the level of service provision within the home health program
- Understands the need to stay current with changes to rules, regulations, standards, and industry trends as they relate to the clinical practice and the operational model of the delivery of home health care. (OASIS, 485 content and coding)
- Assumes overall responsibility for oversight and management of the clinical services provided by the clinical and non-clinical teams
- Understands the need to maintain the necessary staff resources and knowledge to keep the order management process current and in compliance with Hennepin Healthcare policy, Joint Commission, and CMS regulations
- Responsible for developing a system of operation and communication that will indicate a need for a proactive management plan to maintain the referral flow from the primary referral sources
- Demonstrates the ability to collaborate and work with the care providers on the multidisciplinary team in a manner that enhances their ability to provide a high standard of excellence of service to all home health program clients
- Maintains a model of supervision with the team leaders responsible for the care provision that enhances their ability to grow professionally and to consistently improve their skills as mentors to the members of the clinical staff
- Assumes responsibility for development of clinical policy and procedure
- Maintains proactive role with team leaders in assessing the skill level, the staffing model, the caseload mix and the need to interview and hire staff in a manner that allows for continuity of care provision
- Understands the need for the development and ongoing management of a robust quality improvement model that relates directly to the delivery of care, cost management, and the various regulatory requirements noted above
- Receives, investigates, and makes recommendations regarding patient complaints in accordance with agency policy and procedures. Utilizes a data driven model for maintaining operational efficiency as well as quality improvement processes
- Serves as a "subject matter resource/ person" for the team leaders and clinical staff to ensure that the delivery of care remains in sync with the plan of care that is documented within Oasis
- Conducts regularly scheduled meetings with clinical staff
- Participates in the development of education for clinical staff to close knowledge gaps and facilitate excellence in clinical service delivery including, but not limited to, the development and performance of annual competency testing that will enhance their performance as care providers
- Utilizes a data driven model for maintaining operational efficiency as well as quality improvement processes
QUALIFICATIONS
Minimum Qualifications:
- Bachelor's Degree in Nursing
- Five (5) or more years of demonstrated leadership, preferably home care including supervision and mentoring of staff
- An approved equivalent combination of education and experience
- Master's Degree in Nursing or leadership
- Demonstrated management ability and knowledge of nursing practice, applicable rules and regulations pertaining to home care
- Strong written and verbal communication skills
- General computer skills and a working knowledge of MS Office, Outlook and the internet
- Demonstrated skills in leadership and workload management
- Strong skills in communication, teaching, and negotiation
- Demonstrated ability to use available resources creatively
- Ability to be flexible and adjust to ongoing changes in health care systems
- Ability to work with people of different races and cultures
- Ability to work well as a team leader/member
- Ability to speak and write the English language in an understandable manner
- Possession of a valid license as a Registered Nurse
