Reports To: Hospice Medical Director

Status: Exempt (Salaried)

FT, PT, PRN: Full-time

Shift: 40 hours; Monday - Friday (or option for four 10-hour days)

On Call Rotation: Phone call 1 weekend/every 6 weeks

Location: Visit patients in the communities of our North service region that need in home and nursing facility visits - travels to Buncombe, Haywood, Madison, and McDowell counties

Position Summary:

Work in collaboration with a Four Seasons physician and within an interdisciplinary team to provide patient care consultation, evaluation, and treatment to patients with serious illness in multiple community settings, including patient homes and long-term care facilities.

Essential Duties & Responsibilities:

• Work to achieve performance goals, pursue progress toward individual learning plan and demonstrate Four Seasons values and competencies
• Provide direct patient care consultations, to include participation in patient and family counseling and management of symptoms related to life-limiting conditions in collaboration with the interdisciplinary team of physicians, nurses, social workers, and chaplains
• Serve as an attending provider and take call for hospice patients
• Perform face to face encounters as needed
• Create and maintain accurate patient records
• Teach and mentor nurses and other staff to obtain quality care outcomes, efficiently with excellent patient and family experience
• Practice within the scope of license, certification, or registration, where applicable, and as required by law and regulations
• Serve as clinical resource for staff of Four Seasons, staff at facilities served, and providers in the community
• Maintain current medical education and appropriate licensure as per policy and State regulations
• Perform other duties as requested or assigned

Minimum Qualifications:

Education:

• Master's degree

Experience:

• Two years of nurse practitioner clinical experience is required

Licensures & Certifications: (if applicable)

• Current NP Board Approved Certification
• Current DEA Registration in good standing
• Meet and maintain eligibility requirements to be appointed medical staff and qualify for privileges at partner facilities
• Valid driver's license and proof of automobile insurance.
