We are seeking a Hospice Nurse Case Manager to be responsible for identifying patient/family needs and for providing skilled nursing assessment, planning and supportive care and appropriate services in accordance with the attending physician's plan of treatment and orders. Service areas include Walnut Creek, Concord, Pleasant Hill, Martinez, Brentwood, Pleasanton, Livermore, Hayward, San Lorenzo, Sunol, Union City, Fremont.

Essential Duties & Responsibilities include (but are not limited to):
- Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process. Maintain a caseload and productivity per agency's protocol.
- Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Maintain regular communication with Director of Patient Services/designee to review caseload.
- Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.
Education/Experience:
- One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing.
- Experience in acute care/ home health and/or hospice setting preferred
Certificates, Licenses, Registrations:
- Current license as a registered professional nurse in the State of California.
- Public Health Nursing certification highly desirable.
- HPNA certification preferred.
- Current California Driver's License and automobile insurance that meets State of California requirements, if driving a car.
Pay Range:
$60.00 - $74.00 hourly
