HOSPICE PHYSICIAN - Position Summary

The Hospice Physician creates and maintains the medical component of every patient's plan of care. The Hospice Physician is responsible for developing a comprehensive plan of care for each patient and ensuring that every member of the hospice team, caregivers, and the patient's family, are knowledgeable of the care plan.

HOSPICE PHYSICIAN - Essential Duties & Responsibilities

Patient Eligibility
  • Certify patient eligibility for hospice care upon admission and at the beginning of each benefit period, ensuring that all legal and clinical criteria are met.

Attending Physician Role
  • Serve as the attending physician for patients without a primary physician at their request or at the request of their existing attending physician. In this capacity, provide all legally required orders for care, including signing the death certificate.

Medical Availability
  • Act as the attending physician as requested in the absence of other attending physicians, ensuring the availability of 24/7 medical care and consultation for all patients and interdisciplinary team members.

Care Coordination
  • Coordinate medical care to ensure competent and appropriate delivery of hospice services, collaborating with interdisciplinary team members and attending physicians to resolve care conflicts and ethical dilemmas.

Interdisciplinary Collaboration
  • Participate in the interdisciplinary development, evaluation, and revision of patient care plans, ensuring that each patient's needs are met comprehensively and compassionately.

Patient Visits
  • Make medically necessary visits in person or via telehealth (when appropriate), and assist in face-to-face encounters as needed to assess and address patient needs.

Leadership in Pain and Symptom Management
  • Provide leadership in the delivery of state-of-the-art pain and symptom management, ensuring patients experience the highest level of comfort and care.

Policy Development
  • Participate in the development of standing orders, clinical protocols, policies, procedures, forms, quality improvement monitors, and service programs as requested.

Advocacy and Representation
  • Effectively represent the hospice program through regular communication with physician colleagues, acting as an advocate for hospice care and seeking opportunities to promote the program.

Community Education
  • Participate in the presentation of community education programs for healthcare professionals and the public, promoting understanding and awareness of hospice care.


HOSPICE PHYSICIAN - Qualifications/Experience
  • Board certified or board eligible in Hospice and Palliative Medicine, preferred
  • Licensed by the Commonwealth of VA as a Doctor of Medicine or osteopathy.
  • Demonstrated willingness and ability to participate in direct clinical practice.
  • Minimum of 2 years' experience required.
  • Demonstrated understanding of hospice philosophy and pain and symptom management.
  • Contribute as part of a collaborative team.
  • Strong communication and interpersonal skills
  • Proficient computer skills and electronic medical records required.
  • Must be able to travel throughout the territory as needed.
