Job Title: Hospice Registered Nurse (RN)
Job Specialty: Hospice Care
Job Duration: 13 weeks
Shift: Days, 5x8 Hr
Guaranteed Hours: 40 hours
Experience: Minimum of one (1) year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health.
License: Current license to practice nursing in the state of employment.
Certifications: BLS (American Heart Association), Current automobile insurance, and valid driver's license.
Must-Have:
- Knowledge of terminally ill patients and their families, hospice care, and special needs of hospice patients and families.
- Excellent patient assessment skills.
- Knowledge of community resources and general nursing practices.
- Ability to operate nursing assessment tools, computers, and standard office equipment.
Job Description:
- Provide direct patient care services to hospice patients and supportive care to families.
- Manage a team consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
- Assure the highest quality of care is provided and documented in accordance with the Plan of Care.
- Perform initial and ongoing assessments of patients.
- Develop and implement the Hospice Plan of Care with other members of the Interdisciplinary Group.
- Report any change in the patient's condition to the attending and hospice physicians.
- Maintain accurate and updated medical records.
- Practice infection control measures and standard precautions.
- Participate in the Quality Assessment and Performance Improvement Program.
- Possibly participate in the agency on-call schedule.
- Promote company core values and complete required compliance annual training.
- Attend staff meetings and maintain a high level of knowledge regarding pain and symptom management.
- Conduct routine supervisory visits and review patient clinical records.
- Maintain contact with the patient's family for support and continuity of care.
- Educate the patient, family, or caregivers regarding the Plan of Care.
- Supervise Aides and prepare written assignments for them.
- Perform bi-weekly visits to the home to assess care and services provided.
