Job Description
Job Title: Hospice Registered Nurse
Job Specialty: Clinical Operations
Job Duration: 13 weeks
Shift: Monday to Friday, 8 a.m. to 5 p.m.
Guaranteed Hours: 40 hours per week
Experience: Minimum of one year as an RN, preferably in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered as experience in individual cases.
License: Current license to practice nursing in the state of employment
Certifications: Basic Life Support (BLS) from the American Heart Association
Must-Have:
- Knowledge of terminally ill patients and their families
- Understanding of hospice care principles
- Excellent patient assessment skills
- Proficiency in general nursing practices
- Knowledge of managed care principles and regulatory guidelines (e.g., Medicare, Medicaid)
- Effective communication and documentation skills
- Ability to operate nursing assessment tools and use computers and other office equipment
- Own transportation for work with a valid driver's license and auto insurance
Job Description:
- Provide direct patient care services to hospice patients and supportive care to their families.
- Manage an interdisciplinary team consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
- Implement the Hospice Plan of Care as prescribed by the physician.
- Document the highest quality of care in accordance with the Plan of Care.
- Perform initial and ongoing patient assessments.
- Develop and implement comprehensive physical, psychosocial, educational, emotional, and spiritual services.
- Report changes in the patient's condition to physicians.
- Maintain accurate and updated medical records and documentation.
- Practice infection control measures and standard precautions.
- Participate in Quality Assessment and Performance Improvement Programs.
- May participate in an agency on-call schedule.
- Promote the company's core values consistently.
- Attend staff meetings and complete required training.
- Maintain updated knowledge related to pain and symptom management in the dying process.
- Make routine supervisory visits and conduct reviews to ensure quality service.
- Serve as the primary liaison between physician, patient/family, and hospice team.
- Educate patients, families, and caregivers about care needs, including medication administration and the hospice philosophy.
- Supervise Aides and prepare written instructions for them.
- Make supervisory visits to patients' residences biweekly to assess Aides' service provision.
