Job Description
Job Title: Hospice Registered Nurse
Job Specialty: Hospice Care
Job Duration: 13 weeks
Shift: Days, 5x8 Hr
Guaranteed Hours: 40 per week
Experience: Minimum one year as an RN in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered as experience in individual cases.
License: Current license to practice nursing in the state of employment
Certifications: BLS (American Heart Association), Current automobile insurance and valid driver's license
Must-Have:
- Knowledge of terminally ill patients and hospice care
- Excellent patient assessment skills
- Knowledge of community resources
- Good oral and written communication skills
- Ability to operate nursing assessment tools, computers, and office equipment

Job Description:
- Provide direct patient care services to hospice patients and support for the patient's family.
- Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff, enhancing hospice care quality.
- Assure the highest quality of care is provided and documented according to the Plan of Care.
- Perform initial and ongoing assessment of the patient.
- Develop and implement the Hospice Plan of Care with the physician and interdisciplinary group.
- Collaborate with the team to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
- Report patient condition changes to the Attending and Hospice Physicians.
- Maintain accurate and updated medical records with interdisciplinary notes, Plans of Care, and assessments.
- Practice infection control measures and standard precautions.
- Monitor staff scheduling and participate in the Quality Assessment and Performance Improvement Program.
- May be required to participate in on-call schedules.
- Promote company core values and complete required compliance training.
- Attend staff meetings and maintain knowledge of pain and symptom management in the dying process.
- Conduct routine supervisory visits with team members monthly.
- Review and follow up on patient clinical records to ensure quality services, adherence to standards of practice, and policy compliance.
- Maintain contact with the patient's family to provide information, support, and continuity of care.
- Maintain primary control and professional management of each patient, acting as the primary liaison between physician, patient/family, and hospice team.
- Educate patients, families, or caregivers based on the Plan of Care, including medication administration, dressing changes, oxygen administration, and hospice philosophy.
