Job Title: Hospice Social Worker MSW
Job Specialty: Clinical-Allied
Job Duration: 13 weeks
Shift: Monday to Friday, 8 am to 5 pm
Guaranteed Hours: 40 hours per week
Experience: One year as a Social Worker in a healthcare and/or hospice setting is required; three to five years preferred.
License: Licensure as required by the state in which the hospice is located.
Certifications: BLS Certification.
Must-Have: Current automobile insurance and valid driver's license.
Job Description:
Provide psychosocial support to patient and family units based on assessments that identify needs, goals, interventions, and services, in accordance with the established plan of care.
Perform psychosocial assessments, case management, education, and ongoing evaluation of patients and families.
Complete comprehensive psychosocial assessments within five days.
Maintain appropriate clinical records and complete documentation by the next business day.
Develop a written plan of care to guide other interdisciplinary team members regarding social work services.
Participate in team decisions regarding patient admission and discharge to hospice services.
Assist with discharge planning, coordinating, and ensuring that plans are followed.
Provide social work expertise regarding changes to patient care levels.
Document findings, needs, problems, measurable goals, plans, interventions, and patient responses in clinical records.
Communicate plans and changes to physicians and other team members through care plans and coordination notes.
Offer casework, evaluation, and service management of patients needing information about family relationships, illness meaning, cultural and spiritual attitudes, long-term care planning, and advanced care planning.
Monitor patient environment safety, including reporting policy on potential abuse/neglect and suicide, and adjust patient care in the home.
Demonstrate teamwork and effective communication to achieve patient, team, and company goals.
Share expertise and initiate interdisciplinary referrals, including community outreach concerning end-of-life care issues.
Educate team members and patient families about community resources and how to access them appropriately.
Negotiate and represent patients and families concerning community resources.
Participate in training, in-service education, and lead modules related to the social work role in the hospice process.
Assist in developing, implementing, and evaluating ongoing quality improvement processes.
Act as a field placement supervisor for social work students, including day-to-day supervision, evaluation, project management, and coordination with university placement coordinators.
Supervise Bachelor's degree Social Workers per state and company policy.
May participate in on-call rotation for admissions, deaths, and crisis management.
Complete required compliance annual training.
Support the company's core values consistently.
Preferred Skills & Experience:
Experience in hospice and understanding the psychosocial dynamics of illness, loss, and death.
Knowledge of community resources and good oral and written communication skills.
