Job Title: LPN Full-Time Evenings
Job Specialty: Long-Term Care (LTC) Licensed Practical Nurse (LPN)
Job Duration: 13 Weeks
Shift: Full-Time Evenings (5x8 per week with potential for 3x12s), Every Other Weekend/Holiday Required
Guaranteed Hours: Not specified
Experience: 1 year as an LPN
License: LPN Licensure
Certifications: BLS (must be through AHA or RQI)
Must-Have: Ability to provide age-appropriate care to adult and geriatric patients
Job Description:
- Demonstrate knowledge and skills necessary to provide care appropriate to patients' age on the assigned unit.
- Provide direct and indirect skilled nursing care for residents under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
- Collect data on residents' physical and psychosocial conditions and communicate findings to the RN and the Interdisciplinary Team.
- Contribute to the development of culturally relevant care plans reflecting nursing diagnoses and interventions.
- Recommend revisions to care plans after evaluating resident outcomes and assess the need for referrals.
- Maintain current knowledge and competency in nursing practice through continuing education and in-service programs.
- Participate in the mentoring/orientation of staff and demonstrate leadership within the professional practice setting.
- Ensure safety, cost-effectiveness, and quality of care while performing required tasks and considering potential exposure to bloodborne pathogens.
