Summary:

Responsible for the day to day coordination of patient care within an assigned team of Registered Nurses. Responsibilities include assessment of referrals, case management, maintenance of standards of practice, patient assignments, staff development, quality assessment, and improvement and communication with all members of the interdisciplinary team, in accordance with the policies and standards of WCC and regulatory agencies. Models appropriate behavior as exemplified in the Mission, Vision and Values.

REQUIRED:

Completion of an accredited nursing program.

Must have at least two (2) years of experience as a Registered Nurse.

R.N. with current license to practice as a professional nurse in the state of employment.

Key Job Responsibilities:
  • Coordinates department operations including scheduling, patient assignments, monitoring department activities and collaborates with other departments to ensure effectiveness of operations and the delivery of quality patient care.
  • Coordinates and ensures patient assessments are performed accurately and timely.
  • Coordinates and develops a prioritized holistic plan of care based on a comprehensive assessment, desired outcomes and currently knowledge.
  • Coordinates and implements the plan of care for patients, either directly or by delegation.
  • Ensures patient plan of care is evaluated on a continuous basis.
  • Communicates with patients and families and other health team members.
  • Demonstrates leadership and promotes professional practice of members of nursing care team.

Supervision Provided by this Position:

Functions in a lead role for Registered Nurses and directs/delegates patient care activities.

Physical Demands/Conditions:
  • Frequent invasive & noninvasive patient contact
  • Exposure to patient body fluids as well as exposure to hazardous or poisonous materials.
  • Ability to react quickly to emergency situations.
  • Ability to handle and administer narcotics as needed.
  • Normal hearing; normal vision, including color, night and peripheral vision; Good hand-eye coordination
  • Ability to lift and carry up to 50 lbs. without assistance and push and pull up to 250 lbs.; Ability to stand, walk, climb stairs, sit in one place, squat, kneel, reach, bend, crawl and twist throughout the shift; Ability to perform repetitive hand and wrist motion as well as using hands for gripping and squeezing; Ability to use hands and feet together for repetitive, coordinated motions
