Our patient-centric Primary Care clinics are dispersed across Minneapolis/St. Paul, MN. We are thrilled to serve the Twin Cities metro in our clinics located in Highland Park (St. Paul), Crystal, and South Minneapolis and Rosedale, with our newest clinic in Eagan launching December 2024. About the Lead Registered Nurse Role:

Reporting to the Sr. Director of Clinical Operations, the Lead Registered Nurse (RN) at Herself Health will provide virtual, patient-centered care to women ages 65+ within a value-based care model. In addition to providing transitional care management (TCM) and triage, the Lead RN serves as a. mentor, trainer, and supervisor for the RN team. They ensure high-quality patient care while managing team performance, approving time off, and collaborating with leadership on process improvement initiatives.

This is a virtual role, working standard business hours in the Central Time zone, Monday - Friday 9:00am - 6:00pm.

Key Responsibilities:
  • Transitional Care Management (TCM)
    • Conduct outreach during the 30-day post-discharge to reduce ER visits, hospitalizations, and readmissions
    • Review daily ADT feeds and initiate patient contact within 2 days of discharge
    • Schedule and coordinate follow-up PCP visits within 7 days
    • Ensure post-discharge services (e.g., PT, home health) are arranged
    • Educate patients on self-management, perform medication reconciliation, and monitor adherence
    • Document all patient interactions in the EHR system
  • Triage
    • Respond to patient inquiries, refill requests, and health messages
    • Use critical thinking skills and nursing judgement to triage patient concerns based on evidence-based protocols
    • Provide medication refills, titrations, and at-home treatment guidance
    • Educate patients on test results, conditions, and procedures
  • Leadership & Team Management
    • Support onboarding and ongoing training of RN team members
    • Conduct regular check-ins to review processes, troubleshoot issues, and ensure best practices are being followed
    • Service as a clinical resource for complex cases or questions
    • Monitor RN team performance, provide coaching and feedback to ensure quality care delivery
    • Approve time-off requests and ensure adequate staffing coverage

As an early member of the interdisciplinary team at Herself Health, the Lead RN may have expanded responsibilities over time and should be comfortable with ambiguity.

Must-have Qualifications and Skills:
  • Active and unrestricted Minnesota RN license required (or ability to obtain by start date)
  • Minimum of 3 years of RN experience in a clinical or telephonic case management role
  • Prior leadership experience (e.g., team lead, preceptor, or supervisor role)
  • Strong knowledge of Medicare, NCQA regulatory requirements, and transitions of care best practices
  • Proficiency in EHR documentation, data tracking, and reporting
  • Exceptional communication and mentorship skills
  • Ability to problem-solve and implement team improvements

Nice-to-have Qualifications and Skills:
  • Multi-state compact license preferred
  • Experience in value-based care, triage, or transitional care management preferred
  • Familiarity with care coordination platforms and analytical tools
