Level of Care Assessor
JOB SUMMARY
The Level of Care Assessor is certified by the state to perform a comprehensive assessment that determines if an individual meets a nursing home level of care and qualifies for PACE services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Evaluates potential new enrollees, determines levels of care, communicates with the appropriate team members. Assessments are conducted in the home of the potential enrollee. Coordinates level of care determinations with the Department of Health and Human Services personnel. Successful completion of the annual state required certification for the LOC Assessor Enter evaluation data into the State electronic system Effectively communicate to IDT evaluation findings Provide comprehensive insight to medical conditions and current medication regimen Effectively communicate PACE services Completes monthly LOC assessments as directed by supervisor Identifies care needs Identifies participant's goals and preferences Assist in outlining an interim care plan Independently schedules appointments with participants Documentation is concise, accurate, and meaningful
REQUIREMENTS
Bachelor's degree in a health-related field is State dependent; professional licensure/certificate may be required. Current RN license in good standing to practice in the required state Hmong Speaking preferred Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance. Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
