We are currently seeking a licensed Physical Therapist to join our skilled nursing rehabilitation team. As a PT in our facility, you'll play a critical role in helping residents regain strength, mobility, and independence. This position is ideal for clinicians who are passionate about geriatric care, functional mobility, and helping patients achieve meaningful goals in a post-acute setting.
You'll work closely with a team of Occupational Therapists, Speech-Language Pathologists, Nursing Staff, and Physicians to deliver therapy that is compassionate, coordinated, and clinically effective.

Job Duties:
- Conduct comprehensive evaluations to assess patients' physical function, mobility, balance, strength, and pain levels
- Develop and implement individualized care plans focused on improving function, reducing risk of falls, and enhancing quality of life
- Provide hands-on physical therapy interventions including therapeutic exercises, gait training, transfer training, and pain management techniques
- Monitor, assess, and document patient progress using facility-approved EMR systems
- Educate patients, families, and caregivers on techniques to promote safety, recovery, and self-sufficiency
- Collaborate with other members of the interdisciplinary care team to ensure cohesive, patient-centered treatment approaches
- Adhere to all federal, state, and facility regulations and standards of care
- Participate in discharge planning, safety assessments, and equipment recommendations
Requirements
- Doctorate or Master's degree in Physical Therapy from an accredited program
- Current New York Physical Therapist license (or eligibility to obtain)
- CPR/BLS certification
- Strong communication and interpersonal skills
- Ability to adapt treatment plans to suit patients' evolving needs and functional levels
- Compassion for working with older adults and individuals with complex medical conditions
- 1+ year of experience in a skilled nursing, rehab, or long-term care setting
- Familiarity with EMR systems such as PointClickCare or Casamba
- Knowledge of Medicare and Medicaid documentation and reimbursement requirements
- Interest in fall prevention, pain management, or neurorehabilitation in geriatric populations.
