Job Description
Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None
Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty
License: Must have a valid LVN/LPN license in hand
Certifications: CPR, BLS required
Must-Have:
- Proficiency in HomeCare HomeBase EMR, used within the last 6 months
- Reliable transportation, valid driver's license, and agency-required liability insurance
Job Description:
- Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.
- Competently apply the nursing process in patient care settings.
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings as required.
- Assigned between 5-10 patients per day, equating roughly to 6-8 points, with a 30 points weekly productivity expectation.
- Types of care include Start of Care, Wound Care, and Follow-up Visits.
- Must be willing to travel up to 75 miles from home clinic; mileage reimbursed.
- Local candidates accepted at the same rate.
- Schedule expectation is M-F 8 am-5 pm, with potential on-call rotation 1-2 times per week.
- All documentation must be completed within 24 hours of the patient appointment.
- Responsibilities may include floating to a nearby clinic to pick up additional hours if needed.
- Familiarity with the Oasis system is required for RNs, PTs, and OTs.
