Job Description
Job Title: Licensed Vocational Nurse / Licensed Practical Nurse
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None; weekly productivity points of 30 required
Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience
License: Must have license in hand
Certifications: CPR, BLS
Must-Have:
- Proficiency in HomeCare HomeBase EMR, used within the last 6 months or completion of a retraining course
- Reliable transportation, current driver's license, and auto insurance
- Ability to travel up to 75 miles from the home clinic
Job Description:
- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies
- Responsible for supervision and evaluation of Home Health Aides and other Licensed Practical/Vocational Nurses
- Conduct home visits and attend periodic office meetings
- Treat an average of 5-8 patients per day with a workload equating to 6-8 productivity points
- Manage types of cases such as Start of Care, Wound Care, and Follow-Up Visits
- Complete all documentation within 24 hours of patient appointment
- Floating options to nearby clinics if patient census is low
- Proficiency in OASIS required for RNs, PTs, and OTs
Additionally, candidates should be comfortable driving longer distances between patient appointments and must accept every patient assigned regardless of distance. There are no weekend or on-call requirements for this position. All applicants must not be currently employed by any affiliated location. Local candidates are accepted at the same rate as travelers.
