Job Description
Job Title: Licensed Vocational Nurse / Licensed Practical Nurse
Job Specialty: Home Health
Job Duration: 13 Weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience
License: Must have a current license in hand
Certifications: CPR, BLS
Must-Have: Reliable transportation, a current driver's license, and agency-required liability insurance
Job Description:
- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies and procedures.
- Supervise and evaluate home health aides and fellow nurses.
- Conduct home visits, requiring up to 75 miles of travel from the home clinic.
- Maintain proficiency in HomeCare HomeBase EMR; complete a retraining course if used outside the last 6 months.
- Ensure documentation is completed within 24 hours of patient appointments, as delayed/incomplete documentation can impact billing and patient care.
- Handle a varying caseload of 5-10 patients daily, targeting 30 productivity points per week.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Engage in periodic office meetings.
- Must be open to on-call rotations, one weekend per month.
- Reimbursement provided at the IRS standard mileage rate for patient home-to-home travel.
