Job Description
Job Title: Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 16:30)
Guaranteed Hours: Facility can call off up to one shift per pay period (every two weeks)
Experience: 2 years of skilled home health experience required; First-timers not accepted
License: Pending license accepted
Certifications: BCLS
Must-Have:
- Own transportation is required
- Oasis experience is required
Job Description:
- Provide skilled nursing care to patients in their homes
- Shift involves visiting up to 6 patients per day achieving 30 productivity points standard
- No weekend or on-call requirements
- No floating between units required
- Open to local candidates within a 50-75 mile radius at the same rate
- System organizes client visits to minimize driving and windshield time with occasional maximum radius of over 50 miles
- Facility requires primary COVID vaccination series and one booster
- Required charting includes EPIC/Oasis
- Any color scrubs are acceptable attire
- All travelers are expected to float between all facilities within a given state. Refusal to float may result in the termination of the assignment and/or forfeiture of guaranteed hours.
- Facilities will supply communication devices needed for the role at no cost, or reimburse work-related mobile phone use up to a maximum amount
Orientation: 40 hours
Modules: Non-billable, average 16 hours spent, over 20 hours requires approval. Completion is self-reported and recorded in transcripts.
