Job Description
Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: No
Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty
License: Must have a valid license in hand
Certifications: CPR, BLS
Must-Have:
- Proficiency in HomeCare HomeBase EMR
- Experience with OASIS within the last 6 months or completion of a retraining course if applicable
- Reliable transportation, current driver's license, and agency-required liability insurance
Job Description:
- Provides, coordinates, and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conducts home visits and attends periodic office meetings.
- Manages an average of 5-8 patients per day, equivalent to roughly 6-8 points, with a total of 30 points per week.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Will travel up to 75 miles from the home clinic with mileage reimbursed at the IRS standard rate from patient home to patient home.
- Involves visiting patients in both care facilities and their homes.
- Requires documentation to be completed within 24 hours of the patient appointment.
- May include on-call duties, which vary per location and profession.
- The candidate should be open to travel, as the position may involve drives longer than 20 minutes between patients.
- Local candidates are accepted at the same rate, with an understanding of responsibilities within the mileage radius.
- In the event of low patient census, there is an opportunity to float to a nearby clinic to pick up hours.
