Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience
License: Must have a valid license in hand
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase EMR; recent experience in the specialty
Job Description:
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Maintain reliable transportation, a current driver's license, and agency-required liability insurance.
- Handle # of patients per day: 5-8.
- Assignments equate to approximately 6-8 productivity points per day, with a weekly expectation of 30 points.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Be open to both facility and in-home patient visits.
- Travel territory may be up to 75 miles from the home clinic, with up to 350 miles per week.
- On-call rotation: One week, Monday to Friday, every three weeks.
- Complete all patient documentation within 24 hours of the appointment to ensure efficient billing and continued patient care.
- Be open to being floated to a nearby clinic if patient numbers are insufficient to meet financial needs.
- Dress Code: Scrubs
- Travelers must be comfortable accepting all patient assignments, regardless of travel distance between appointments.
