Job Description
Job Title: Licensed Practical Nurse (LPN)
Profession: Nursing
Specialty: Home Health
Duration: 13 Weeks
Shift: Day
Hours per Shift: 8 Hours (08:00 - 17:00)
Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty.
License: Must have license in hand.
Certifications: CPR, BLS
Must-Have: Recent specialty experience required.
Description:
Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures.
Utilizes the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.
Home visits required.
Periodic office meetings required.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day: 5-8.
Contractor will be assigned between 5-10 patients per day.
Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
Weekend requirements vary by location and profession.
Traveler will be compensated for travel from patient home to patient home.
Local candidates accepted at the same rate.
All documentation must be completed within 24 hours of the appointment with the patient.
Delayed and incomplete documentation may negatively affect billing and follow-up patient care.
There are no guaranteed hours with this contract.
Traveler must achieve weekly productivity points, with a total of 30 points required to equate to 40 hours per week.
Participants must be open to traveling up to 75 miles from the assigned clinic.
All travelers must be proficient with specified EMR systems.
This role requires acceptance of patient schedules regardless of distance.
