Manager, Care Navigation (local remote)

Primary Duties and Responsibilities:
The Manager, Care Navigation, is responsible for managing and optimizing the work of the supportive clinical and care coordination services offered to our populations, including oversight of pharmacy, nursing, and social work services offered to support populations with specialty needs occurring during a transition in care. The Manager ensures excellence in patient care coordination while managing staff productivity. This role works to identify, implement and evaluate key business processes to enable the success of Tandigm clinical programs.
This position is a key member of Tandigm leadership, working closely with Complex Care and High Value Network leaders, and is an important contributor to the success of Tandigm programs through active participation in interdisciplinary planning.
Responsibilities include but are not limited to management of operations, process evaluation and staff supervision and support. Direct reports include pharmacists, field-based nurses, facility-based nurses, and post-acute coordinators who provide care coordination services telephonically, in homes, and in facilities.

Essential Functions
- Consistently exhibits behavior and communication skills that demonstrate Tandigm Health's commitment to superior customer service, including quality, care, and concern for all internal and external customers.
- Evaluates individual teammate performance throughout the year, ensuring transparency for each team member on their individual performance, including conducting 1:1 meetings at least monthly.
- Responsible for facilitating and/or developing on-going departmental training.
- Takes responsibility for effective implementation of pharmacy, nursing and social policies, procedures, guidelines and programs related to field-based services and medication management.
- Supports the development and implementation of process to control quality, utilization and cost
- Leads all quality related initiatives, including but not limited to pharmacy related Stars and HEDIS measures
- Identifies underlying drivers of quality performance and develops and implements plans to improve quality performance, with emphasis on medication management and reconciliation interventions, where appropriate.
- Uses, protects, and discloses patients' protected health information (PHI) in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
- Performs additional duties, as assigned.
Required Qualifications:
Minimum
- Graduate of an accredited school of nursing or pharmacist license required
- Master's degree from a four-year college
- 3 years minimum healthcare leadership experience, preferably in population health, hospital, home care or pharmacy services.
- 5 years or more of healthcare leadership experience with a multi-disciplinary team, preferably in population health, hospital, home care or pharmacy services.
Licensure:
Current Pennsylvania RN license in good standing or valid pharmacist license.
Knowledge, Skills, Abilities
- Strong clinical judgement ability to support decision-making, problem-solving, and patient safety concerns
- Effective communication and relationship building to ensure patient-centered care and team collaboration
- Knowledge of medication management and reconciliation interventions
