This is a leadership role focused on optimizing patient outcomes, ensuring regulatory compliance, and supporting effective hospital throughput.

Key Responsibilities:
- Oversee the daily operations of the case management department, including staffing, scheduling, attendance, and staff support.
- Monitor case management processes to ensure alignment with established policies, professional standards, and regulatory requirements.
- Lead the coordination of care planning, discharge planning, and continuum of care processes.
- Support timely patient discharges to promote optimal length of stay and reduce readmissions.
- Foster strong relationships with healthcare providers, payers, physicians, and patient families.
- Perform daily rounding to provide leadership and consultation to case management staff.
Qualifications:
- Education:
- Bachelor's Degree in Nursing (BSN) required
- Master's Degree in Nursing (MSN) preferred
- Licensure:
- Active California Registered Nurse License
- Experience:
- Minimum 5 years of case management experience and/or nursing management in an acute hospital setting
- In-depth knowledge of case management standards, hospital reimbursement models, and payer systems (e.g., Medicare, Medi-Cal, Managed Care)
- Familiarity with InterQual and M&R utilization review tools
