Job Summary and Qualifications

The Pharmacy Clinical Manager is responsible for designing, implementing and managing the pharmacy clinical programs. This includes formulary management, policy and procedure development and education across the healthcare spectrum. The Clinical Manager shall also assist in managing the pharmacy when the Pharmacy Director or designee is unavailable.

Major Responsibilities:

  • Interprets orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system
  • Accurately compounds and dispenses pharmaceuticals
  • Complies with facility policy and procedures and regulatory requirements
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
  • Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed
  • Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection
  • Supports participation in daily multidisciplinary and bedside rounds (may need to attend)
  • Identifies, designs and analyzes drug utilization reviews to promote rational drug therapy and determines action plan based on results
  • Provides accurate and timely medication information to healthcare staff and patients
  • Develops, manages and mentors pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as determined appropriate based on patient population and literature guidelines
  • Develops, manages and mentors for the antimicrobial management program (AMP)
  • Documents all clinical activities and interventions accurately and completely
  • Responsible for mentoring and education as well as ongoing analysis of interventions to insure accurate and appropriate documentation and provides summary reports
  • Supports and participates in the goals of the organization's quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives
  • Is responsible for design, analysis and reporting of adverse drug reactions, medication errors and pharmacy quality initiatives and mentors others in their roles in these programs
  • Develops and actively participates in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians and maintains responsibility for assessment of participants in these programs
  • Develops and documents staff competency on an ongoing basis and develops improvement plans when deficiencies in competencies are identified
  • Primary preceptor for pharmacy students and/or residents as it relates to clinical pharmacy services
  • Responsible for designing and managing program and mentoring student/resident as well as other preceptors
  • Maintains professional competency sufficient to meet current and future needs of the facility
  • Develops and manages specialty pharmacy practice programs as related to clinical pharmacy services
  • Provides basic technical support for automated dispensing machines and other pharmacy computer systems
  • Effectively and proactively intervenes in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions
  • Trains and participates with Disaster Team drills and real-event response as assigned
  • Trains and participates with Code Teams as assigned and obtains and maintains BLS-ACLS or PALS current certification as appropriate
  • Maintains appropriate controlled substance records
  • Works with the Pharmacy Director to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards
  • Complies with all regulatory standards
  • Works with the Pharmacy Director and Operations Manager to identify and develop processes to meet or exceed all regulatory standards and participates in quality and regulatory reviews and audits
  • Prioritizes work and leads others to comply with facility medication turn-around time policy and procedure and identify areas for improved efficiency
  • Supervises and assists pharmacy technicians and other support staff effectively
  • Works with the director to establish pharmacy goals and objectives and is responsible for implementation and monitoring
  • Participates with various nursing and medical staff committees as necessary
  • In conjunction with pharmacy director and operations manager, is responsible for staff performance evaluations
  • Acts as manager in charge as assigned
  • Is responsible for development and review of physician order sets and other evidenced based medication use guidelines
  • Is responsible for routine publication of the pharmacy newsletter and/or other forms of written and electronic communication
  • Is responsible for drug class reviews and drug monographs
  • Is responsible for managing and monitoring the drug formulary
  • Is responsible for the development, implementation, and monitoring of pharmacy clinical initiatives and shares pharmacy operation responsibility with Pharmacy Operation Manager
  • Is responsible for the development and review of pharmacy policies and procedures
  • Provides oversight of staff scheduling
  • Participates in the design and update of pharmacy computer system programs relating to pharmacy, nursing and physician programs
  • Participates in the maintenance of pharmacy dictionaries including adding new drugs to the drug dictionary and Gold Standard Formulary Charge Master maintenance
  • Maintains professional competency sufficient to meet current and future needs of the facility
  • Develops and manages specialty pharmacy practice programs as related to clinical pharmacy services
  • Performs other duties as assigned
  • Practices and adheres to the "Code of Conduct" and "Mission and Value Statement"


What qualifications you will need:

  • B.S. or Pharm D. from an ACPE-accredited School of Pharmacy Required
  • Doctor of Pharmacy from an ACPE-accredited college of pharmacy Preferred
  • Minimum of 1 year as a Pharmacy Clinical/Operations Specialist Required
  • At least 5 years of hospital pharmacy experience required; preferably within area of focus Required
  • Completion of hospital PGY2/ pharmacy specialty residency or fellowship Preferred
  • Specialty BCPS certification Preferred
  • Other certification (e.g. FCCP, FASHP) Preferred
  • Hospital Pharmacy Management experience Preferred
  • Experience performing basic mathematical calculations, balance and reconciliation of figures Required
  • Experience working in Microsoft Office, Meditech, Business Objects and purchasing vendor applications Required
  • ACLS certification Preferred
  • Active Pharmacist license in good standing with the appropriate State Board of Pharmacy Required
  • Active Pharmacy Consultant license, or equivalent as per State requirements for a Pharmacy Required
