Job Title: MDS Coordinator
Job Specialty: Nursing
Job Duration: 12 months
Shift: 8:00am - 5:00pm, Monday through Friday
Guaranteed Hours: 40 hours per week
Experience: Minimum years of experience not specified
License: RN nursing licensure for state of practice is required
Certifications: Must be certified as a Resident Assessment Coordinator (RAC-CT) within 6 months of hire
Must-Have: Proficiency in MDS, knowledge of the RAI/MDS process, graduate of an accredited school of nursing
Job Description:
- Completes the assigned portions of the MDS for Nursing and signs the forms to signify completion of the process.
- Maintains residents' self-esteem, privacy, and confidentiality of personal information.
- Assists in maintaining safe, pleasant, orderly resident living areas.
- Completes the nursing portion of MDS and reviews for completeness and validity.
- Queries other team members for the accuracy of MDS and provides final quality checks prior to MDS transmission.
- Observes and reports residents' conditions and communicates with physicians using center systems.
- Directs and assists with coworker, resident, family/significant other education.
- Provides support and recognition to coworkers related to the completion of the RAI/MDS process.
- Uses the EMR accurately and effectively communicates in English, both orally and in writing.
