JOB SUMMARY

The Medication Therapy Management (MTM) and Float Pharmacist is responsible for optimizing therapeutic outcomes through improved medication use and education for patients of our health centers. This pharmacist works under the Director of Pharmacy. They will maintain the daily business operations of a pharmacy department in the absence of a staff pharmacist and will float to any of our centers to staff a pharmacy department as needed. He/She is responsible to compound and dispense prescribed medications, drugs, and other pharmaceutical items for patient care by performing duties in both the adjudication and production work environments while providing direct supervision of Pharmacists, Pharmacy Services Representatives, Pharmacy Technicians, and Pharmacy Support Specialists performing pharmacy related duties.

PRIMARY FUNCTIONS
  • Perform comprehensive medication therapy reviews of eligible patients to identify, prevent, and resolve medication-related problems, including adverse events, and minimize cost
  • Utilize a patient's medical record to perform chart reviews and reconcile drug therapy
  • Conduct medication adherence phone calls to engage patients with upcoming prescription refills or past due fill dates to enhance compliance with medication therapy
  • Identify and address patient-specific barriers to medication adherence and disease state management
  • Conduct one-on-one patient counseling sessions and medication reconciliations
  • Provide disease-state specific education and training to patients, providers, and clinical staff
  • Collaborate with True Health providers to resolve medication-related issues and optimize drug therapy for patients
  • Recommend evidence-based and patient-centered medication treatment plans to providers
  • Promote cost-effective drug therapy recommendations and alternatives, as well as evaluate the appropriateness of high-cost pharmacotherapy
  • Communicate to providers and staff the savings opportunities available at True Health pharmacies
  • Develop MTM summary reports to provide to patients and providers as appropriate
  • Review and monitor MTM-related reports, including HEDIS/STAR reports
  • Participate in all assigned meetings to address patient care and operational requirements, including one-on-one communication
  • Assist with supervision and development of pharmacy staff
  • Meet with providers, medical staff, and administrative staff to keep them updated on pharmacy operations, updates, pricing changes, drug availability, etc. when requested or necessary
  • Help develop and deliver presentations to staff members as assigned
  • Provide support for new pharmacy department initiatives
  • Contribute to projects as needed to support business needs
  • Assist the Director of Pharmacy in the handling of any reporting and auditing requirements
  • Maintain operations of the pharmacy departments as assigned
  • Dispense and verify prescriptions as required
  • Follow all medication guidelines as well as policies and procedures for receiving, filling, dispensing, logging, maintaining loss prevention procedures, and avoiding diversion for all inventory including controlled medications
  • Ensure that pharmacy policies as well as federal and state laws and regulations in all prescription related issues, including 340b, HIPAA, SOP, QA, and CQI, are followed
  • Review and verify prescriptions issued by physicians or other authorized prescribers to assure accuracy, appropriateness, and determine formulas and ingredients needed
  • Compounds medications, using standard formulas and processes such as weighing, measuring, and mixing ingredients, if applicable
  • Provide counseling on both prescription medications/items and over the counter medications/items per company policy and as required by state and federal law
  • Ensure that all controlled drug policies and procedures as required by state, federal, and organizational guidelines are followed
  • Performs trouble shooting functions for third party discrepancies and other prescription filling issues
  • Answer the telephone and assist at the front counter to maintain the workflow of the pharmacy.
  • Manage and supervise pharmacy staff including: any staff pharmacists and pharmacy technicians
  • Responsible for ensuring that all required pharmacy, pharmacist, and pharmacy technician licenses/certifications/registrations are current and valid as required by state, federal and pharmacy guidelines.
  • Maintain contact with providers, clinical support staff, and administrative staff to determine needs, resolve problems independently, improve processes and promote effective drug therapy.
  • Stay current with new and changing drug therapies and uses
  • Responsible for the proper maintenance of pharmacy drug inventories
  • Responsible for the proper storage of all pharmacy inventory
  • Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.
  • Maintains a clean, neat, and orderly pharmacy department at all times
  • Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals as needed
  • Actively engages patients and clients with respect to customer service programs offered by the pharmacy in an effort to grow pharmacy business and volume of prescriptions dispensed
  • Actively engages patients and clients with respect to program integrations offered by the pharmacy
  • Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs
  • Enters all data accurately such as patient name, prescribed medication, and cost, to maintain pharmacy files, charge system, and inventory
  • Maintains awareness of third-party Rx systems and resolves third party rejection problems promptly, and resubmits rejected claims for payment on a timely basis
  • Resolves and reports prescription errors immediately and adheres to company policy regarding medication incidents and continuous quality improvement
  • Maintains awareness of loss prevention controls and procedures to identify and minimize profit loss.
  • Develop and maintain a good working relationship with physicians and other health care professionals in order to better assist the patients
  • Delivers accurate reports and assignments on time and meets assigned deadlines
  • Complete all assigned projects and tasks accurately and on time
  • Responsible for the success of any department inspections
  • Must have reliable transportation and be able to travel to and staff the pharmacy at other health center locations when necessary
  • Participate in events outside of normal course of business as assigned


  • Ability to work any work schedule as assigned by the organization which may include evening and/or weekend hours
  • Must be able to provide support outside of regular business hours as required, including the ability to be on call if needed
  • Provide department access outside of regular scheduled hours when needed in regard to the pharmacy department alarm system as well as maintenance that needs to be performed within the department
  • Assist with emergency preparedness and reopening procedures as needed
  • Any other duties and tasks may be assigned (may include technician-related duties depending on available staff)

  • EDUCATION AND EXPERIENCE
  • Graduate of an accredited college of pharmacy: Bachelor of Science in Pharmacy or Pharm. D. degree required
  • Minimum of 5 years of management experience and practice in a retail type community pharmacy required
  • Minimum of 3 years of MTM experience preferred
  • Current Florida Pharmacist License, active and in good standing, with a current Immunization Certification
  • BLS CPR Certification required and maintained current
