POSITION PURPOSE
The Medical Director (MD) will lead and provide clinical oversight for our telemedicine-based population health services. This role is central to a team-based, patient-centered care model that addresses chronic, acute, preventive, and sometimes palliative care needs of adult patients across diverse communities. The Medical Director will oversee clinical care provided remotely via our telemedicine platform and will work closely with in-field nurses to manage a panel of patients through both virtual interactions and coordinated in-person visits.
This position combines clinical leadership, telemedicine expertise, and collaborative skills to drive high-quality outcomes through technology-enabled, team-based care. Our infrastructure provides comprehensive support, including EHR, telemedicine training, patient recruitment, administrative services, and robust care coordination.
As a senior leader, the MD will oversee a team of clinical professionals and collaborates with executive peers to establish an integrated, patient-centered approach to healthcare delivery.
This is a hybrid position, that will primarily operate in a remote capacity but may require occasional travel to our Phoenix office or other satellite locations, based on business needs.

DUTIES & RESPONSIBILITIES

Clinical Oversight and Strategy
- Lead the telemedicine clinical team, providing guidance on managing chronic disease, preventive care, and acute and palliative needs via virtual platforms.
- Shape the organization's approach to remote patient management, ensuring that the model meets best practices and regulatory standards for quality of care.
- Develop and implement strategies to optimize the telemedicine care model, with an emphasis on patient engagement, self-management skills training, and continuous quality improvement.
Patient Care and Clinical Management
- Oversee a team of primary care providers who manage adult patient panels, ensuring effective telemedicine-based diagnosis, treatment, and follow-up.
- Collaborate with field-based nurses who conduct home visits, administer follow-up care, and assist patients in self-management and chronic disease management.
- Engage in regular case reviews with providers and nurses to ensure comprehensive, coordinated, and patient-centered care.
Team Leadership and Development
- Mentor and support the telemedicine clinical team, including primary care providers and nurses, fostering a collaborative environment focused on high-quality care delivery.
- Facilitate regular training sessions and provide resources to enhance team proficiency in telemedicine care delivery, patient engagement, and chronic disease management.
- Ensure clinical teams are supported with ongoing access to resources, technology, and best practices in telehealth and population health management.
Program Development and Quality Improvement
- Design and implement protocols for managing chronic, preventive, and acute care in a virtual environment, tailored to the needs of diverse patient populations.
- Partner with quality and data teams to assess program outcomes, leveraging data analytics to identify areas for improvement in care delivery and patient outcomes.
- Monitor compliance with telemedicine standards, state regulations, and organizational policies, and lead quality improvement initiatives for remote care.
Collaboration and Coordination of Care
- Work with administrative teams to support seamless scheduling, billing, patient record retrieval, and rapid check-ins between visits, ensuring a streamlined experience for patients.
- Collaborate with field nurses to coordinate timely in-home visits, follow-up care, and engagement with patients to facilitate self-management.
- Engage with stakeholders, including health systems and community organizations, to expand telemedicine services and improve accessibility for underserved populations.
Telemedicine Platform Management
- Ensure the telemedicine platform operates efficiently and meets clinical needs, providing input on updates, functionality, and user training.
- Monitor provider and patient feedback on the telemedicine platform to drive ongoing improvements and ensure high satisfaction with virtual care.
- Serve as a subject matter expert on telemedicine best practices, coaching providers and staff on effective remote communication, digital health engagement, and troubleshooting.

- Attend regularly scheduled meetings.
- Compose and share reports of Team KPIs.

SKILLS & QUALIFICATIONS
- 5+ years of experience in primary care or population health, with telemedicine or virtual care experience strongly preferred.
- Proven experience in a leadership role, ideally within a telemedicine or remote care environment.
- Background in managing chronic conditions, preventive care, and population health management.
- Proficiency with telemedicine platforms, EHR systems, and data-driven decision-making.
- Strong ability to lead and collaborate in a virtual care setting with remote teams and patients.
- Excellent communication skills to effectively engage patients, coordinate with field nurses, and provide virtual care.
- A patient-centered approach and commitment to advancing health equity in underserved communities.
EDUCATION, LICENSES, & CERTIFICATIONS
- Active and unrestricted medical license in California.
- Board certification in a relevant clinical specialty (e.g., Family Medicine, Internal Medicine, Preventive Medicine).
- Certification in Telemedicine or Population Health (e.g., CPHQ, Certified Professional in Healthcare Quality) is helpful.
- Certification in Population Health Management is helpful.
- Lean Six Sigma or similar process improvement certification is helpful.
