Job Description: The Primary Health Care (PHC) Counselor reports directly to the Primary Health Care Manager-Central. Professional peer review and responsibility will be maintained through a joint accountability to Mental Health/Addictions through the Manager of the Clinical Team and Manager of Crisis Assessment Team. The PHC Counselor is an integral member of the primary health care team, in a collaborative, co-location model. As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for ongoing mental health and psychosocial services for their clients. The services to be delivered through this position include screening and assessing the needs of referred clients and determine a relevant treatment plan which may include short term intervention with the counselor and/or case management or referral to other community programs. The PHC Counselor initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. He/she promotes quality health care outcomes working within established standards of practice to ensure that clients receive a high standard of primary health care. This will be done in accordance with Sunrise Health Region mission statement and Integrated Primary Health Services policies and procedures of the facility and department.

- Bachelor of Science in Nursing (BScN)
- Bachelor of Science in Psychology
Other Education and Training
- Bachelor of Psychiatric Nursing is acceptable
- Bachelor of Social Work is acceptable
