Job Description
Job Title: Rehabilitation Therapist
Job Specialty: Physical Therapist, Home Health RN
Job Duration: 13 weeks
Shift: Day-Time 8-5, possible on-call
Guaranteed Hours: 40 hours per week
Experience: Minimum of 2 years of experience preferred
License: Current license to practice as a Physical Therapist
Certifications: Valid driver's license and appropriate auto insurance
Must-Have:
- Bachelor's degree or higher from an accredited Physical Therapy program
- Ability to perform interventions as allowed under the physical therapy practice act
Job Description:
- Responsible for evaluation and rehabilitation intervention in the home setting
- Manage and supervise patient care along with patient and family education
- Evaluate and establish a plan of care to meet patients' needs in the home environment
- Coordinate care with other disciplines and seek orders for additional services as needed
- Provide direct patient care based on the established care plan and physician orders
- Apply knowledge of physical therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in evaluations, re-evaluations, and treatment techniques
- Modify treatment as appropriate, including frequency and duration of care
- Master scoring of the CMS-required OASIS and collaborate with Quality team for needed adjustments
- Acquire and maintain a basic understanding of insurance requirements in Home Health
- Participate in multidisciplinary conferences to establish appropriate care plans
- Communicate effectively with physicians and other staff members
- Document all skilled services provided to patients, adverse events, and relevant communications in the medical record
- Engage in Quality Assurance Performance Improvement initiatives to assist the Agency in meeting its goals
- Manage productivity standards in line with Agency standards
- Maintain accurate time logs and mileage
- Follow agency policies for start and end of day commuting and logging of mileage
- Communicate with scheduler daily
- Maximize continuity of care and efficient travel routes when preparing schedules
- Oversee services provided by the Physical Therapist Assistant and perform supervisory visits within the required timeframe
- Assume supervision of CNA when RN is no longer involved in the care plan
- Participate in relevant care conferences
- Plan admissions, OASIS time points, and discharges appropriately throughout the patient's episode of care
- Maintain requirements of physical therapy licensure, including continuing education standards
- Uphold ethical behavior standards as defined by the physical therapy licensure board and agency policy
- Participate in agency-provided educational opportunities as scheduled
- Perform other duties as assigned by supervisor
