SUMMARY: Accept patient referrals, and coordinate all aspects of care for Hospital patients, referred by medical staff members. Provide admission, medical management, and transfer/discharge of all patients on the medicine service.
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following:
Accept patient referrals and coordinate all aspects of care for Hospital patients, referred by medical staff members.
Provide inpatient hospital care, including admission histories and physicals, daily rounds / patient visits, procedures, discharges, and other appropriate care in accordance with approved clinical privileges, Hospital and Medical Staff policies, bylaws and rules and regulations. Order appropriate status within the Hospital according to the Two-Midnight Rule and nationally recognized admission criteria. Discuss end of life preferences with patients, family and significant others who need to know. Refer patients to medical specialists, social services or other professionals as appropriate. Conduct timely discharge planning and discharge patients. Coordinate care and communicate frequently with primary care physicians, case managers and other team members as appropriate. Respond to Code Blue/White and other emergencies. Perform daily bedside multidisciplinary rounding on all patients on the Medicine service. Serve as the physician on-call for residents in the Extended Care Center (ECC) during evening and weekend hours or when the ECC Medical Director is on vacation during hospitalist scheduled days. Function as the Medicine Consultant to all areas of the Hospital, including timely consultations to the Emergency Department, surgeon and other Medical Staff members upon request. Along with other members of the Hospitalist team, provide on-call coverage for shifts as assigned, on-site or off-site by telephone, responding within time frames established by the Medical Staff for emergency, urgent and routine requests. The parties acknowledge and agree that the duties set forth in this item 3 constitute one of the essential functions of Physician's job. Physician, along with the other Hospitalists, shall periodically meet to confer upon and set schedules for each shift, days-on, days-on with call and days-off. "Days-on" means that Physician shall be at the Hospital during normal patient care hours and available to provide call coverage for shifts assigned. Participate in appropriate Medical Staff meetings. Work with nursing leadership on development and implementation of evidence based clinical pathways, protocols and other clinical effectiveness projects. Work with hospital staff to ensure full compliance with CMS Conditions of Participation for Critical Access Hospitals (CAHs), core measures and meaningful use requirements. Work with hospital staff to ensure optimal patient satisfaction as indicated on both HCAHPS and internal patient satisfaction surveys. Participate with Discharge Planners and staff in maintaining effective utilization and resource management, medical record appropriateness and appropriate discharge planning. Maintain excellent working relationships with and participate in the implementation and improvement of systems to provide excellent service to patients, physicians and other colleagues and Hospital staff. Participate in the resolution of patient and provider complaints and enhancement of patient and provider satisfaction as measured by periodic surveys. Complete and maintain adequate and proper medical records with respect to all patients examined or treated in accordance with all legal, accreditation, regulatory and third party payor requirements. Complete proper documentation for evaluation and management and procedure coding to allow the hospital to bill for professional fees. Comply with all Medical Staff Bylaws and policies, Hospital and Medical Staff Codes of Conduct, and the Medical Staff Fitness for Duty policy. Demonstrates System Values in performance and behavior. EDUCATION AND EXPERIENCE:
- D. or D.O degree and successful completion of medical residency in Family Medicine or Internal Medicine
LICENSES, CERTIFICATIONS:
Required:
- Current California medical license;
- A current Federal Drug Enforcement Administration (DEA) registration (approved for all drug schedules);
- Board certified or board eligible in Family Medicine or Internal Medicine. If not board certified, must become board certified within five (5) years of residency or fellowship training. Must maintain board certification and satisfy specialty / subspecialty recertification requirements, as applicable.
- Basic Life Support for Healthcare Providers (BLS)
OTHER EXPERIENCE / QUALIFICATIONS:
- Membership on the Hospital's medical staff and appropriate clinical privileges at Hospital in Family Medicine or Internal Medicine
- Able to obtain and maintain professional liability insurance
- Eligible for participation in the Medicare and Medi-Cal programs and for credentialing by contracted health plans, as applicable
- Ability to utilize an electronic medical records system to review patient health history and test results, document clinical activities and findings and place orders
