Job Description: The Primary Health Care Case Manager serves as a single point of access to an integrated health care team, participates in collaboration with the patient, in identifying health needs and coordinates resources toward meeting those needs. Working in a multidisciplinary environment including, but not limited to, Primary Health Care Team members: Family Physicians, Nurse Practitioners, Primary Health Care Counsellor, Home Care, Emergency Medical Services, Pharmacy, Therapy services, Public Health, Dietitians, Psychiatrists, Psychologists, Social Workers, Addiction Workers, and visiting specialists as this position requires networking with other professionals and agencies within the community.

- Bachelor of Science in Nursing (BScN)

- Equivalent combination of education and experience is acceptable

- Demonstrated clinical competency
