Nurse Care Manager

North Suffolk is looking for a Nurse Care Manager (NCM) to join our Community Behavioral Health Center (CBHC) program! The Nurse Care Manager will be the initial contact for patients seeking MAT-PDOA treatment and act as the primary liaison between the patient and the MAT-PDOA medical provider (psychiatrist, psychiatric nurse practitioner) throughout the treatment process. The NCM supervises the initial screening, after which a waivered medical provider sees the patient to confirm the patient's opioid use disorder diagnosis and appropriateness for MAT. The NCM supports the patient through the induction process under the orders of the waivered medical provider. The patient and the NCM remain in contact during the first day of induction for support, education, and titration under the direction of the waivered physician. Initially, the NCM sees patients weekly for required follow-up visits with drug screening and verification of behavioral health counseling. The amount of required follow-up and oversight with the NCM will be dictated by patient need. The NCM is supervised by the MAT-PDOA psychiatrist or psychiatric nurse practitioner as well as the NSCS Medical Director.
Additional Responsibilities:

• Oversight of buprenorphine/naloxone and naltrexone intake assessment, induction, stabilization, maintenance and relapse management.
• Ensures that state and federal guidelines are followed and collaborate as needed with MAT provider, social worker/counselors, psychiatrists, pharmacy, primary care provider, and specialty care providers to whom the patient has been referred.
• Coordinate between MAT provider and pharmacy: obtain medication history, assist with prescription processing and refills, prior authorizations, insurance issues, concerns of diversion, misuse, safety, storage, and behavioral health referrals.

What you'll need:

• Education/Qualifications:
  • Registered Nurse with current Massachusetts license to practice required.
  • Bachelor's Degree in Nursing preferred.
• Skills/Certifications:
  • Initial eight-hour Nurse Care Management training curriculum required (may be completed upon hire).
  • Experience in office-based treatment with buprenorphine/naloxone, including the use of buprenorphine/naloxone for the treatment of opioid use disorder in the office setting.
  • Training regarding the use of naltrexone for the treatment of alcohol use disorder and prevention of relapse to opioid dependence.
• Experience/Knowledge:
  • Knowledge of rehabilitative approaches to the treatment of individuals with severe and persistent mental illness. Knowledge of substance use disorders and treatment approaches.
  • Knowledge of evidence-based practices.
  • Strong communication and liaison skills.
  • Crisis prevention/intervention skills.
  • Ability to work as part of a multi-disciplinary team in providing field based services.
  • Understanding of person-centered, recovery-oriented, trauma-informed approach to treatment.
