In this role, you'll provide clinical case management and health oversight for individuals with I/DD, guiding personalized care plans and supporting Direct Service Providers. You'll play a key part in maintaining quality care, offering training, delegating tasks in alignment with Board of Nursing standards, and advocating for those we serve.
What You'll Do
- Deliver skilled nursing, care coordination, and clinical oversight
- Develop and update individualized care plans based on client needs
- Train and support Direct Service Providers on medical tasks and best practices
- Complete nursing delegation and ensure regulatory compliance
- Collaborate with families, providers, and interdisciplinary teams
- Manage clinical documentation and medication records
Who You Are
- Colorado RN license (required)
- BSN and CPR certification
- 1+ year acute care experience; I/DD or home health a plus
- Strong communication, documentation, and teaching skills
- Bilingual in Spanish a plus
Location & Schedule
- Hybrid: Tu/Th in-office (Lakewood), M/W/F home visits
- Territory: North of I-70 - includes Broomfield, Longmont, Greeley, Thornton, etc.
Pay & Benefits
- $36-$42/hour + mileage
- $5,000 sign-on bonus
