As an Advanced Practitioner, you will play a critical role in delivering advanced clinical care to patients across various settings. You will work collaboratively with physicians, nurses, and other healthcare professionals to provide comprehensive care, including diagnosing and treating medical conditions, prescribing medications, and developing patient care plans. Responsibilities:
  • Conduct comprehensive patient assessments, including medical history reviews, physical examinations, and diagnostic testing, to diagnose and manage acute and chronic medical conditions.
  • Develop and implement individualized patient care plans, including prescribing medications, ordering diagnostic tests, and recommending treatment options, based on evidence-based practices and patient needs.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care, facilitate interdisciplinary communication, and ensure continuity of care across various healthcare settings.
  • Provide patient education on disease management, preventive care, and health promotion, empowering patients to take an active role in their own health and well-being.
  • Perform minor procedures, such as suturing, wound care, and joint injections, as appropriate to the scope of practice and specialty area.
  • Monitor patient progress and make adjustments to care plans as needed, ensuring timely and appropriate interventions to achieve optimal patient outcomes.
  • Maintain accurate and thorough documentation of patient encounters, treatment plans, and patient progress in the electronic medical record (EMR) system, in compliance with regulatory requirements and practice policies.
  • Participate in quality improvement initiatives, clinical research studies, and continuing education activities to enhance clinical practice, promote evidence-based care, and advance the field of advanced practice.
  • Provide leadership and mentorship to other healthcare team members, including nursing staff and medical residents, fostering a collaborative and supportive work environment.
