Position Description
Provides primary care as part of a health care team in an ambulatory setting in a particular discipline. Provides patient education and counseling as well as physical assessment. May participate in teaching programs for students and other student residents. Work is performed under usual clinic conditions and requires a high degree of contact with patients, families, and other clinic personnel and outside professionals. Some positions in the classification require travel to outreach clinics and home visits, and participation in educational efforts and special programs that occur outside of normal working hours.

- Provide primary care as a part of a healthcare team in an ambulatory setting.
- Provide patient education and counseling.
- Conduct patient health and physical assessments.
- May participate in teaching programs and serve as a preceptor for teaching programs for students as identified by the department and supervisor.
- May require travel to outreach clinics and home visits.
- Required to participate in educational efforts and special programs that may occur outside an of normal working hours.
- Ability to communicate effectively with a diverse patient population.
- Required to complete all training required by the institution and department by deadlines identified.
- Required to complete patient charts and enter data into the Electronic Health Records (EHR) meeting deadlines identified by the department.
- Required to attend departmental meetings such as monthly provider meeting in addition to other meetings as assigned.

- This position is subject to the CMS vaccination mandate.
Preferred Qualifications
- Experience as an FNP nurse practitioner.
- Experience in providing primary care in a medical setting, preferably in an ambulatory setting.
- Completion/Graduate of an accredited school of nursing Master of Nursing (MSN) program.
- Certified Family Nurse Practitioner (FNP) or other Nurse Practitioner
- Candidates for the position should have an outstanding reputation in the delivery of primary care services, with significant experience in primary care.
- Experience using electronic health records (EHR) software /systems with experience in documentation in medical records.
- Ability to communicate effectively orally and written with patients, staff, and other entities. Bilingual English/Spanish
- Ability to demonstrate working knowledge of current healthcare practices.

Occasional Duties
- Conduct presentations as assigned by supervisor.
- Attend conferences to complete CEUs as needed.
- Travel to meetings in different locations.
- Attend required meetings.
- Serve as a preceptor to students.

Required Qualifications
- Education: Graduated from an approved accredited nursing program.
- License/Certification: Current RN license in the State of Texas or have the ability to transfer your license to Texas. Certification as a Nurse Practitioner in the appropriate nursing specialty from an approved certification program as recognized by the Texas State Board of Nurse Examiners. Recognition by the Texas Board of Nurse Examiners as an Advance Practitioner.
