Job Description:
  • Facility is seeking an experienced APRN/Nurse Practitioner for our Adult Medicine clinic. The APRN/NP provides comprehensive primary health care, including health promotion and disease prevention, and collaborates with department physicians. The role includes referring patients to specialists as needed and delivering high-quality, culturally competent care.

Responsibilities:
  • Provide comprehensive primary health care, including health promotion and disease prevention.
  • Collaborate with department physicians and refer patients to specialists as necessary.
  • Take medical histories, assess medical conditions, and interpret findings.
  • Communicate effectively with patients, peers, and stakeholders.
  • Maintain accurate and thorough clinical documentation.
  • Make evidence-based clinical decisions and adhere to quality measures.
  • Utilize electronic health records and other computer systems effectively.
  • Perform other duties as assigned.

Required Qualifications:
  • Graduate of an accredited nursing school and completed an approved nurse practitioner program.
  • Valid State of Connecticut licensure.
  • Eligible to obtain DEA and state narcotic registration.
  • Deep knowledge of nursing or physician assistant theory and practice.
  • Proven experience in providing high-quality, professional clinical care.
  • Ability to deliver culturally competent care in a collaborative environment.
  • Skill in medical history taking, condition assessment, and findings interpretation.
  • Effective communication skills, both oral and written.
  • Proficiency in maintaining timely and accurate clinical documentation.
  • Comfortable with electronic health records and computer systems.

Preferred Qualifications:
  • 1-2 years of experience in an ambulatory setting or providing urgent care.
  • Spanish language skills are preferred.
  • APRN or PA License in the state of Connecticut is preferred.
