Description:
Community Health Network, in collaboration with Community Hospital Oncology Physicians, LLC, is seeking an experienced Nurse Practitioner (5 years experience minimum) to collaborate with medical/hematology oncologists.

Desired requirements
Minimum of five (5) years' experience in a clinical setting (beyond training)
Board certification and IN medical state license required
Desire to function in a highly collaborative atmosphere
Consistent and consecutive employment and training history
Excellent communication skills fostering collaboration with all members of care team
Develop and maintain relationships with referring physicians
