Minimum Education/License/Certification Requirements

• Graduate from an accredited program with an advanced degree.
• Must hold an unrestricted license to practice in the State of Indiana.
• Must have two (2) years of experience, neurology experience preferred.
• Be able to obtain prescriptive privileges required and the ability to maintain current privileges (to be allowed prescriptive authority) and exercise a well-written collaborative practice agreement with a designated physician.
