Job Description
JOB SUMMARY: The Nurse Practitioner in the Ambulatory Care works as a provider in collaboration with a licensed physician or physicians, assessing the physical and clinical status of patients by means of interview, health history, physical examinations and diagnostic tests of the patient. In accordance with medical protocols, he/she evaluates test findings, makes initial medical diagnosis and initiates appropriate action to facilitate the implementation of the therapeutic plan consistent with continuing health care needs of the patient. The Nurse Practitioner works within the scope of regulatory agency standards and policies and procedures as set forward by H+H and Gotham Health, Inc./East New York.
RESPONSIBILITIES:
Minimum Qualifications
For Assignment to Nurse Practitioner
1. A valid New York State license and current registration to practice as a Registered Professional Nurse; and
2. Current certificate as a Nurse Practitioner issued by the New York State Department of Education
(NYSED); and
3. A current certification issued by the State Education Department to prescribe medication.
For Assignment to Nurse Practitioner (Primary Care/Behavioral Health)
1. In addition to meeting the qualification requirements of Nurse Practitioner, must be assigned to a
Primary Care or Psychiatric/Behavioral Health setting.

RESPONSIBILITIES:
- Provides general care to medical, pediatric and gynecological patients
- Obtain health history and relevant demographic data from patient, family member or guardian.
- Access physical and psychological health status of individuals and families, through history taking and physical examination.
- Performs complete nursing assessment affording diagnostic priority for physician's intervention.
- Orders routine x-rays and laboratory tests.
- Interprets the result of tests and initiates further action, as necessary.
- Evaluates findings of the health assessment in collaboration with physicians and other health professionals in order to make decisions about treatment.
- Inform patients of treatment alternatives.
- Request appropriate consultations.
- Counsel patients/families or guardians on impact of illness and any modifications needed for health maintenance.
- Provide information to patients, families or guardians relating to health status.
- Order prescription for medication from the formulary list approved by the Chief Medical Officer and on file in the Medical Board Office.
- Make adjustments on medication within accepted corporation protocols, initiates requests for selected laboratory tests and interprets results.
- Initiate referrals to other hospital departments or community agencies as required.
- Collaborate with other members of the interdisciplinary team in order to determine, evaluate and attain health goals.
- Record and maintain a system of patients' progress notes to plan program management and therapy appropriate to the needs of the patient.
- Provide follow-up appointments and track broken appointments in accordance with written policy.
- Provide nursing management follow-up selected groups of patients with chronic illness (e.g. diabetes, asthma or hypertension).
- Initiate and participate in quality improvement programs.
- Provide teaching, guidance and direction to nursing personnel, in rendering health care.
- Participate in health promotion and health fair activities.
- Provide clinical and didactic instruction to professional staff, students and community groups and conducts in-service educational programs.
- Interpret and recommends revision in policies and protocols affecting nurse practitioner services and participates in program evaluation.
- Initiate and participate in studies and research related to nurse practitioner services, healthcare and allied fields.
- Seeks assistance and guidance in performance of duties, as necessary.
