Title:Nurse Practitioner (Hospice)

Department: Home Health & Hospice

FLSA Status:Non-Exempt

Reports To: Director of Patient Care Services

Summary: Conducts face-to-face visits with hospice patients in their homes to assist the hospice team in determining whether the patient is still eligible for continuation of hospice services.

Essential Functions:

1. Assesses patient's physical condition upon recertification and as needed in accordance with physician's orders, plan of care and Self-Help Home Health & Hospice policies and procedures.

2. Initiates communication with attending physician, Hospice Medical Director, and hospice team members as needed to coordinate optimal care and use of resources for the patient and/or patient's family.

3. Obtains data on physical, psychological, social and spiritual factors that may influence patient's or family's health status.

4. Provides a detailed written narrative when clinical findings support a life expectancy of six months or less.

5. Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.

6. Maintains accurate and timely documentation of all care delivered and ensures compliance with local, state, and federal statutes and regulations.

7. Performs other duties as assigned.

Qualifications:

1. Current California Licensed Nurse Practitioner.

2. One year of prior professional nursing experience. Home health, oncology or hospice experience preferred.

3. Must have understanding of hospice principles of care and philosophy and familiarity with hospice requirements.

4. Current CPR certification.

5. Must pass health screening for TB clearance, physical examination and background check.

6. Bilingual in English and Cantonese preferred.

7. Must be able to travel via public transportation or personal vehicle for performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
