ESSENTIAL FUNCTIONS
- Review clinical information, coordinate and oversee patient care in consultation with the palliative medical physician, interdisciplinary team, and other healthcare practitioners.
- Consult with the patient's primary care physician, and other healthcare practitioners regarding the patient's eligibility for palliative care, ongoing care needs, and medical management related to the patient's serious/terminal condition.
- Conduct in-person and telephone visits to assess/reassess patients' need for Palliative Care services and ongoing eligibility.
- Manage health conditions and serious illnesses by developing an individualized plan of care, reviewing medications and/or treatments, obtaining consultations or making referrals, and coordinating palliative services and/or hospice transitions.
- Educate/counsel patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, symptom management, advanced care planning, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients.
- Participate in the Palliative Care IDG.
- Complete required documentation to support Palliative Care eligibility and to meet regulatory requirements for communication of findings to Community Healthcare of Texas, including documentation required for billing in accordance with Medicare and agency guidelines.
- Communicate to Senior Director, Medical Director and appropriate hospital staff regarding any patient needs requiring medical intervention. Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols.
- Coordinate visits and scheduling with Senior Director and Team Assistant.
- Maintain compliance with all regulatory and accrediting agencies
- Demonstrate an appreciation and understanding of the philosophy, mission, and service standards of Community Healthcare of Texas and for providing excellent customer service.
- Liaison with palliative care teams at Texas Health Resources hospitals and participate in any palliative care meetings at the hospitals as requested
- Conduct face to faces if needed for transfer to the hospice product line.
POSITION QUALIFICATIONS
- Licensed as an Advanced Practice Practitioner in the State of Texas.
- At least 2 years of experience in geriatrics or palliative care
- Proficient skill using a computer is required; Microsoft office suite experience is required.
