Founded in the early 1970s by four African American women in Peekskill, New York, Sun River Health was established to address the lack of accessible healthcare in their community. With the help of a small federal grant, the first Sun River Health site opened its doors in 1975. Over the years, the organization has grown into a comprehensive FQHC system with over 45 locations, serving more than 250,000 patients across our service area. In December 2018, we expanded our reach through a merger with Brightpoint Health, an FQHC network providing integrated services in New York City, enabling us to further enhance our ability to deliver patient-centered care to a diverse population.

Position Overview:
Sun River Health is seeking a Nurse Practitioner (NP) for a full-time, 4-day-a-week position at our Fountain House location in New York City. The NP will join a dedicated team that works to provide comprehensive, patient-centered care to individuals across the city. Under the supervision of a physician, the Nurse Practitioner will be responsible for diagnosing, treating, and educating patients while maintaining a focus on prevention and wellness. The Fountain House location offers a collaborative, team-oriented environment where healthcare professionals can deliver high-quality care to those in need.

Essential Duties and Responsibilities:
  • Patient Care & Assessments: Perform comprehensive physical exams, take detailed patient histories, and develop tailored treatment plans. Order appropriate lab tests, draw blood, and perform diagnostic tests (e.g., wet prep, microscopic exams) when necessary. Diagnose and treat minor illnesses, prescribe medications, and consult with supervising physicians for complex cases or referrals.
  • Patient Education & Counseling: Educate patients on preventive care, lifestyle modifications, and disease management to promote health and wellness. Offer counseling on various health issues, including chronic conditions, medication adherence, and managing mental health.
  • Documentation & Compliance: Ensure accurate and timely documentation of patient care and treatment plans in accordance with organizational standards and regulatory requirements. Follow up on abnormal results (e.g., pap smears, STDs) and ensure proper documentation of patient progress and follow-up care.
  • Team Collaboration & Case Conferences: Participate in interdisciplinary case conferences to discuss patient care plans and collaborate with other healthcare providers. Attend team meetings and participate in continuous quality improvement initiatives aimed at improving patient outcomes.
  • Clinical Oversight & Safety: Adhere to OSHA infection control standards and CDC guidelines to ensure a safe and healthy environment for both patients and staff. Provide high-quality care that meets the organization's productivity, quality, and patient satisfaction standards.
  • Fulfilling Special Patient Needs: Provide care across various patient age groups (infants, children, adolescents, adults, and geriatrics), ensuring age-specific care is delivered effectively.

Minimum Education & Qualifications:
  • Education: Master's degree in nursing from an accredited program.
  • Licensure/Certification: Valid Nurse Practitioner license in New York State. CPR/BLS certification.
  • Experience: 5+ years of clinical experience as a Nurse Practitioner, preferably in an FQHC or large medical practice setting. Strong knowledge of diagnostic and treatment procedures, especially preventive care.
  • Specialized Skills: Ability to perform physical exams, manage chronic conditions, and prescribe medications. Experience in patient education, particularly around chronic disease management and prevention. Ability to collaborate effectively with a multidisciplinary healthcare team. Bilingual in English and Spanish preferred but not required.

Position Type: Full-Time (4 days a week)
Salary Range: $145,197.00 - $152,236.50 annually (prorated based on hours worked and dependent on licensure and experience)
