Job Description
Purpose of Position:
Under general supervision and in accordance with New York State requirements for written practice protocols and written agreements or for collaborative relationships with qualified physician(s) or licensed health care facility, serves as a licensed, independent practitioner responsible for planning, coordinating, delivering and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations and nursing standards of care.
Responsibilities:
Minimum Qualifications
For Assignment to Nurse Practitioner
1. A valid New York State license and current registration to practice as a Registered Professional Nurse; and
2. Current certificate as a Nurse Practitioner issued by the New York State Department of Education (NYSED); and
3. A current certification issued by the State Education Department to prescribe medication.
For Assignment to Nurse Practitioner (Primary Care/Behavioral Health)
1. In addition to meeting the qualification requirements of Nurse Practitioner, must be assigned to a Primary Care or Psychiatric/Behavioral Health setting
Department Preferences
Rotating shifts (day, evenings, nights)
EM or Urgent Care experience with FNP preferred
Able to review radiology and lab results and act on them, as well as answer patient questions
Willing to work in the low acuity section of the ED.
Interested in working in ED Follow Up Office
Strongly prefer National Board Certification in Emergency Medicine

In addition to base compensation, you may also receive the following based on the Collectively-Bargained Agreement:
An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below:
An annual certification differential of $1,878 if you currently hold an appropriate certification that is nationally recognized in the discipline or area of specialization for which you are employed.
An annual nursing experience differential ranging from $1,097 up to $9,100 for up to 10 years of experience if you have outside nursing experience as a Nurse Practitioner.
For NP (PC/BH) ONLY: An annual assignment differential ranging from $10,000 up to $25,000 if assigned to a Behavioral Health department AND regularly working evenings/nights

Job Description

Responsibilities:
- Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families.
- Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.
- Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician's intervention.
- Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
- Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.
- Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.
- Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment and other pertinent information. Identifies health and medical risks and needs.
- Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.
