The Division of Cardiology at Mercy Health Services is looking for a passionate Nurse Practitioner to help initiate a Heart Failure Clinic at the Heart Center at Mercy. The Heart Failure NP would be expected to provide personalized care for patients 18 years and older, diagnosed with Heart Failure and experiencing Heart Failure Exacerbations. The services include review of medical history and physical examination, adjustment of cardiac medications as needed, blood work and lab testing, personalized education and support, and follow-up care and collaboration with Mercy's cardiologists. This position would also work closely with hospitalists, Population Health providers and other specialists to ensure seamless care transitions and patient outcomes, including support for admissions, discharges and diuresis treatments.

Responsibilities

  • Managing the patient flow effectively and efficiently.
  • Triages patient calls and accurately assesses patient needs. Accurately documents all correspondence with patient.
  • Assisting in development of treatment/management protocols.
  • Provides treatment and patient care under the supervision of a physician.
    • Focusing on heart failure patients referred for follow-up care from the inpatient services as well as those referred from cardiology and PCP practices.
    • Coordinating care with advanced heart failure services at outside facilities.
    • Coordinating care with Mercy Bridge Clinic for IV therapies.
    • Collaborating with home nurses providing services to heart failure patients.
    • Assisting in the transition to hospice care for appropriate patients.
  • Communicates effectively and efficiently with patients and their families.
  • Assists physician with in-office procedures.
  • Completes disability paperwork in a manner consistent with office procedures.
  • Dictates concise and compete medical records utilizing the departmental dictation format.

Requirements

  • Masters in Nursing: Certified Nurse Practitioner
  • Minimum 5 years of Nurse Practitioner experience preferred
  • BLS/ACLS certification
  • License Requirements
    • The CRNP should be qualified and duly licensed and registered to practice medicine in Maryland and shall have current federal and Maryland narcotics numbers
    • Required CRNP
    • Required Healthcare Provider Level CPR
  • Must be computer literate or learn within 6 months.
  • Must be able to read, write, and speak the English language in an understandable manner.
  • Must be able to work independently and possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to effectively communicate and deal tactfully with other personnel, visitors, patients, physicians, customers, and the general public.
  • Must be flexible, have personal integrity, and the ability to function effectively as a team with other employees.
  • Must be able to follow written and oral instructions.
