Responsibilities
Will work out of their Creston Iowa location, providing care to patients in home/nursing facilities in the surrounding area, mostly with elderly populations. Travel is required using personal vehicle between locations and homes. Must have a valid driver's license, auto insurance, and reliable transportation, with mileage reimbursement provided. Typical hours are 7:30 AM to 4 PM, with flexibility for varying schedules and programmatic requirements. Latest appointments typically end by 5 PM.
Pay and Benefits
The pay range for this position is $95517.00 - $114620.00/yr.

Responsibilities
- Provide comprehensive Occupational Therapy services to enhance functional abilities for daily self-care tasks.
- Evaluate patients, create treatment plans, and communicate with patients, families, and care teams.
- Document evaluations, daily notes, progress notes, plans of care, and discharge summaries timely.
- Collaborate with the rehabilitation team to coordinate treatment programs.
- Develop home programs and treatment protocols using evidence-based guidelines.
- Monitor patient progress and adjust treatment plans as needed.
- Train and evaluate therapists, supervise occupational therapy assistants.
- Adapt to changing patient schedules and maintain positive working relationships.
- Degree in Occupational Therapy
- Valid Iowa Occupational Therapy license
- Current Healthcare Provider CPR certification
- Mandatory Reporter training certification (within 30 days of hire)
- Ability to work independently and exercise sound decision-making
- Ability to accept and work with diverse populations, culturally, economically, and educationally
- Valid driver's license, auto liability insurance, and reliable transportation
