General Summary:
The Oncology Clinical Pharmacy Specialistis involved with the care of cancer patients throughout their treatment. The Pharmacist will work with other members of the healthcare team to ensure treatment plan accuracy and optimization of supportive care measures. The Oncology Clinical Pharmacy Specialist will serve as a subject matter expert for the Oncology Fellows during their training to help prepare them for a future in oncology.
Responsibilities:
1. Establish a collaborative practice program with oncologists within identified areas of need including supportive care measures for patients receiving chemotherapy on an inpatient and outpatient basis.
2. Conducts staff development programs or educational services to pharmacy staff, nursing staff, Oncologists, and other healthcare professionals.
3. Serves as a subject matter expert in oncology, completing drug information questions for the oncology service line, reviewing Electronic Health Record care plans to ensure they are consistent with current national guidelines.
4. Round with the Oncology Fellows, Nurse Practitioners, and Physicians to optimize therapy according to established national evidence-based guidelines and Bayhealth policy. This includes but is not limited to making recommendations regarding correct medications, monitoring outcomes, dose adjustments, and supportive care measures.
5. Provides patient specific medication education using teach back method for new oral oncolytics and for intravenous chemotherapy as needed.
6. Review oncology care plans within the Electronic Health Record. This includes new care plans as requested by Oncologists and a continual review of existing care plans for updates and appropriateness.
7. Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data.
8. Serves as a preceptor to pharmacy residents and students
9. Actively participate in Oncology Service Line meetings to work towards improving safety, efficacy, and quality of cancer care at Bayhealth.
10. Promote and demonstrate rational, cost-effective drug therapy. Focus on cost avoidance and cost savings due to medication use within the Oncology Service Line.
11. All other duties as assigned within the scope and range of job responsibilities

Required Education, Credential(s) and Experience:
- Education: Doctorate Degree
; Pharmacy
; Post graduate Year 1 Residency or equivalent experience (at least 3 of clinical pharmacy practice) OR post graduate year 2 residency in Oncology or equivalent experience (at least 3 of oncology practice) - Credential(s): Licensed Pharmacist
Board Certified Oncology Pharmacist
; - Experience:
Required: Three years of hospital experience and completion of clinical training and three years of training in oncology.
Preferred: Completion of PGY-2 ASHP-accredited Pharmacy Residency in a related area of expertise.
Completion of PGY-1 ASHP-accredited Pharmacy Residency and a minimum of 3 years of experience in a related area of expertise AND BPS board certification in Oncology
