Four Corner's Regional Health Center
The Four Corners Regional Health Center is located in the beautiful high-desert climate of the Four Corners area - where the four states of Arizona, New Mexico, Colorado, and Utah adjoin. The health center is part of the Shiprock Service Unit and opened in January 2007. FCRHC provides ambulatory care services including family medicine, internal medicine, mental health, physical therapy, optometry, dental, social services, pharmacy, x-ray, laboratory, public health nursing, diabetes education, health promotion/disease prevention.

The Health Center serves approximately 7,000 Native Americans, of which the majority are members of the Navajo Nation. Strong Navajo cultural traditions exist within the community, offering an opportunity to learn the Navajo language, or to learn about traditional Navajo medicine.
Recreation opportunities exist such as camping, fishing, hiking, biking, backpacking, snowshoeing, snowboarding, cross country skiing, and alpine skiing. Telluride, Durango, Moab, Canyonlands, Grand Gulch, the Colorado River, the San Juan River, several large lakes, and Colorado Wilderness areas are just short drives away.

Position Overview
Professional optometry services are to be performed in the Optometry Department of the Four Corners Regional Health Center/Shiprock Service Unit/Navajo Area Indian Health Service (NAIHS), located in Red Mesa, AZ.
The Optometrist shall provide professional optometry direct patient care services as follows:
  • Work under the general technical guidance of the FCRHC Chief Medical Officer (or his designee) and the Chief of Optometry.
  • Optometry staff members shall be subject to Medical Staff Ongoing Professional Practice Evaluation and Focused Provider Performance Evaluation as specified in the Shiprock Service Unit Policy & Procedure, Focused Provider Performance & Ongoing Professional Practice Evaluation.
  • The Optometrist shall provide direct care to patients to include: patient evaluation, diagnostic testing, clinical management, diagnostic and therapeutic procedures, pharmacotherapy and consultation with/referral to appropriate specialty services, as needed.
  • The Optometrist shall serve as a consultant to physicians, nurses, and other staff as well as other community agencies and service providers, as needed.
  • Shall complete appropriate medical records for all patients seen to document pertinent clinical data necessary to provide comprehensive eye care; complete clinical reports and correspondence concerning patients under his/her care to physicians, hospitals, and other individuals and facilities in a timely fashion. Clinical data reports for the records of each patient shall be maintained in compliance with JCAHO standards.
  • Shall provide direct patient care services and appropriate, timely medical services under the terms of this contract in accordance with the PWS PCP FCRHC SEPT 2017 9 standards of care established by recognized medical care organizations and in accordance with policies and procedures of Service Unit's Medical Staff Bylaws, Rules and Regulations, including attendance at department and staff meetings.
Requirements:
  • Doctoral of Optometry Degree from a school accredited by the Accreditation Council on Optometric Education (ACOE).
  • Active unrestricted Optometrist License in any U.S. State or Territory that allows for treatment of glaucoma
  • Therapeutic Pharmaceutical Agents (TPA) and Diagnostic Pharmaceutical Agents (DPA) certified.
  • Must have a minimum of 12 months experience providing direct clinical care in the preceding two years
