We are partnering with North Carolina DHHS to operationalize a statewide Medicaid Plan designed to support Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system so that they receive seamless, integrated, and coordinated health care. Within the Children and Families Specialty Plan (CFSP), and regardless of where a member lives, they will have access to the same basic benefits and services, including Physical health, Behavioral health, Pharmacy, Intellectual/Developmental Disabilities (I/DD) services, long term services and supports, Unmet health-related resource needs, and Integrated care management. We envision a North Carolina where all children and families thrive in safe, stable, and nurturing homes.
The Innovations and Outcomes Monitoring Manager is responsible for leading initiatives to improve and innovate programs under the Child and Family Services Plan (CFSP). This role focuses on ROI and clinical outcomes-based impact of the $74M portfolio for CFSP, drives cost of care initiatives, collaborates with internal and external partners to gather and interpret comprehensive data, and uses insights to drive continuous improvement and new program development for diverse CFSP populations, including foster care, biological families, DSS-involved families, adoption assistance, juvenile justice, and former foster care. This role will be the Plan point of contact for the state-driven Outcomes and Evaluation Workgroup.
This role will collaborate extensively with various teams within the Elevance Health structure, including Cost of Care, Quality, Data Analytics, HIP, and NC CarePoints Connect. The primary responsibility is to ensure the alignment of initiatives and projects across the NC CFSP portfolio of programs and investments to effectively meet the needs of specialty populations. This position will reside in North Carolina, and the internal company title is Program Director.

Requires a BA/BS and minimum of 10 years experience in external client facing program management, project management; complex business processes, strategic and business planning; or any combination of education and experience, which would provide an equivalent background.

- Develop and direct the proposed HBCT Outcomes Monitoring and Evaluation Program.
- Develop and manage, in collaboration with the NC DHHS agencies, including DSS, DHB, DMH and DPH - the proposed outward-facing HBCT Outcomes Monitoring Oversight Workgroup (CFSP) including HBCT clinical and non-clinical leadership, HBCT clinical and non-clinical staff, NC DHHS and key Divisions (including data groups), DPS/JJ, DPI, Network providers, SMEs, Members, Advocates, Key informants with lived experience.
- Lead the creation and implementation of strategic initiatives aimed at reducing and optimizing costs within child welfare services. This includes identifying opportunities for cost savings while ensuring high-quality care and services.
- Support the development of performance improvement initiatives unique to the CFSP population.
- Collaborate with partners to design new initiatives that support children, families, and communities. Monitor targeted outcomes identified by the Department/other key entities, to guide development of new initiatives on state-focused goals, such as SDoH, permanency, and kinship care.
- Support collaboration with the Department and key State entities, NC Medicaid Plans, and other key stakeholders, addressing data access/management/analysis; data-driven services planning; Key state initiatives and innovation opportunities.
- Support collaboration with the Department and key State entities, NC Medicaid Plans, and other key stakeholders to track long-term, cross-systems outcomes (clinical and non-clinical) among individuals who exited foster care.
- Support and participate in strategies to communicate key findings and conclusions to the Department and key divisions, staff, network/providers, care managers, delegated care managers, trainers, members and families, and other key stakeholders.
Requires a BA/BS and minimum of 10 years experience in external client facing program management, project management; complex business processes, strategic and business planning; or any combination of education and experience, which would provide an equivalent background.
Highly preferred:
- Ph.D within a health-related field with a strong background in clinical outcomes.
- Expertise in developing, directing, monitoring, and evaluating programs, particularly in a healthcare setting.
- Expertise with clinical data sets and analysis, directing monitoring and evaluation programs, particularly in a healthcare setting.
- Familiarity with public and behavioral health metrics as they relate to child welfare, child well-being, child and family functioning, safety, permanency, and social determinants of health.
- Proven ability to work effectively with a wide range of stakeholders, including government departments, healthcare providers, and community advocates.
- Strong ability to convey complex data and findings to various audiences, ensuring clarity and understanding.
- Degree in Information Technology, Computer Science or related field of study with experience in information technology, healthcare, consulting, and/or consumer technologies.
