Role Scope:
This is an opportunity for a mission critical leader. The Outreach & Enrollment Manager is responsible for closely developing and managing key community relationships, as well as a team of Habitat Health Outreach Specialists and Enrollment Specialists. Critical job responsibilities will include sourcing and maintaining local outreach channels for prospective participants, ensuring prospective participant enrollment success, and managing a team's individual performance targets on a monthly basis. This manager will be responsible for leading new participant growth efforts for the Sacramento center location, including ensuring future prospects have a comprehensive financial and functional assessment and meet all compliance requirements by DHCS and CMS to enroll in our PACE program. The Outreach & Enrollment Manager is expected to review metrics daily, support growth initiatives, coach staff members and generally ensure the Outreach and Enrollment team operates smoothly and successfully in collaboration with the Sacramento Center's Executive Director and Habitat Health's Growth leadership.

We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected base salary range for this position is $85,000 to $120,000 + bonus. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.

Core Responsibilities & Expectations for the Role
- Manage and develop a team of quota-carrying Outreach/Enrollment Specialists & Enrollment RNs
- Carry and deliver against targets for monthly team contribution to participant growth
- Recruit, retain, and nurture your team of sales professionals with high potential and strong performance history
- Coach direct reports in successful identification of new prospects and successful enrollment opportunities, as well as career planning/development
- Provide skill training and mentoring on business account engagement and prospect engagement, including consistent and regular ride-alongs with Outreach Specialists and supervision of eligibility assessments conducted by Enrollment Specialists & RNs
- Provide an unmatched experience for participants and prospects
- Work closely with and maintain strong relationships between clinical and operations leadership & staff within the center
- Lead by example for your outreach team, encouraging solutions-based growth strategies
- Develop both the long-term vision and strategy for the team and manage day-to-day growth team operations
- Be accountable for hitting center patient acquisition targets and driving accountability within your team
- Be an ambassador in the local community to help engage future prospects - building relationships with other healthcare providers, social/community organizations, and marketing partners
- Share best practices for prospect and business account engagement
- Other duties as assigned
Required Qualifications:
- Minimum of 5 years in a sales management role or other comparable management position.
- Strong computer skills
- Ability to create and analyze excel spreadsheets
- Experience managing through a CRM system
- Analytical approach toward sales data & performance
- Comfort with personal sales quota in addition to management responsibilities
- Excellent relationship and people management skills with the ability to motivate a team
- Strong focus on data and analytics to drive targeted activities and return on growth efforts
- Ability to manage competing priorities while maintaining a positive attitude
- Inclination to work in a results-oriented role
- Willing to work weekends as necessary, particularly for community events
- Proof of valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law.
Preferred Qualifications:
- Familiarity with the PACE program or other elderly care organizations
- Deep understanding of the local Sacramento health care community
- Bilingual: English & Spanish (preferred), Mandarin or Vietnamese
