- Proficiency in English (Speak, Read, Write)
- Ability to work nights and/or weekends as required
Job Description:
- Function independently to provide high-quality, patient-centric care in various settings through different modalities, including in-person, telephone, video, or secure email.
- Gather patient history, conduct physical exams, review prior records, and analyze ancillary data using sound judgment and up-to-date medical information.
- Develop and monitor appropriate treatment plans; communicate effectively with patients, families when appropriate, and care team.
- Provide efficient and compassionate care utilizing a multi-disciplinary approach, maintaining continuity, coordination, and evaluation.
- Educate and counsel patients and families to support the care plan, treatment, and discharge processes.
- Collaborate with healthcare team members, leadership, and medical staff to ensure safe patient care and communicate pertinent findings to the physician and team.
- Assist collaborating physicians with patient management in the hospital, adhering to care site policies, regulations, and approved Scope of Practice.
- Ensure compliance with regulatory standards and practice metrics, documenting in accordance with regulatory and care site guidelines.
- Adhere to the mission, vision, and values of the employing organization, abiding by service behavior standards.
- Perform other duties as assigned and may float to other departments based on business needs. May also be placed on-call during regular shifts.
